We appreciate your co-operation in returning them before your appointment. This will facilitate your intake and enable your practitioner to spend less time asking questions and so devote more of your visit to treatment and education. These forms are extensive, so we encourage you to set aside 10-15 minutes to fill them out. You can return the forms by email, fax, or in person to reception.
